$250,000

Initiative Summary & Purpose

HealthNet’s Homeless Initiative Program (HIP) runs a variety of programs, from a housing triage line and street outreach to Rapid Rehousing programs with intensive case management as well as providing medical services to those in shelters and on the streets. The project at the center of this funding request is the Diversion and Rapid Exit program.

Vision

HIP’s Diversion and Rapid Exit programs seek to create a scaled, collaborative, and coordinated model of Housing problem-solving infused with flexible funding to:

Divert households as quickly as possible from housing instability with cost-effective, participant-centered solutions to spare the additional trauma experienced with literal homelessness that interrupts each individual’s ability to thrive

Improve access to safe, affordable housing, mental health services, and substance abuse prevention for households in crisis

Connect participants in need of mental health and substance use treatment to quality services in Marion County

Reduce stress on the overburdened homeless services system in Marion County which predominantly serves marginalized populations

By improving access to resources and care needed for homelessness, mental health, and substance use crisis prevention, Holistic Housing will serve as a catalyst for our community’s collective efforts to create systems of care that better serve vulnerable people and that effectively address the intersections of housing and health.

Target Funding Audience

Our target audience for funding of HIP’s Diversion and Rapid Exit programs would include organizations that are passionate about finding flexible, person-centered solutions to homelessness. These organizations recognize it costs less to address housing instability when it occurs than to rehouse someone who has lived unsheltered for an extended period of time.

Immediate Funding Need

We are seeking support to continue our Diversion and Rapid Exit programs after December 2024. These programs have three areas of financial need:

  • Client assistance funding to provide short-term financial assistance to people experiencing housing instability
  • Staffing: diversion specialists, housing specialists and administrative/logistical/management support
  • Materials and training
